New Patients
We look forward to meeting new patients and want you to be comfortable during your visit and provide you with the best possible care. To help us do that, we ask our patients to follow the guidelines below.
New Cardiology Patients:
If you are scheduled to see a Cardiologist, you will receive a new patient packet in the mail. Included is a letter confirming your appointment, directions to our offices, physician bio sheet and new patient information form. If there is not enough time to mail a packet before your appointment, you will be asked to arrive prior to your scheduled appointment so that you can complete the necessary paperwork.
You will also receive a telephone call one or two business days prior to your appointment to confirm the date and time.
Please bring to your appointment the following:
- a current list of all your medications and dosages, including vitamins and herbal supplements or bring the medicines;
- a current copy of your insurance and/or Medicare card;
- any medical records you have from your primary care physician (if you need a referral or prior authorization, please call your primary care physician to obtain one prior to your appointment); and
- the completed new patient information form.
When you arrive at our office, please check in with the receptionist. If you have a co-pay, payment is expected at the time of check in. The receptionist will ask you to sign a HIPPA Notice of Privacy Act form. We make every effort possible to keep your appointment on schedule and appreciate your patience when unavoidable delays occur. Once you have been assigned to an exam room, a Medical Office Assistant will obtain your height and weight, as well as review your medications. You will then see your physician. If the physician recommends any testing, it will be scheduled prior to leaving the office.
If you cannot make your scheduled appointment, please call our office as soon as possible. In most cases we can offer your appointment to another patient.
New Pulmonology Patients:
If you are scheduled to see a Pulmonologist, you will receive a telephone call one business day prior to your appointment to confirm the date and time.
Please bring to your appointment the following:

A nurse will take you to the exam room, where your height and weight will be recorded and a review of your medications taken. Once the physician has examined you, if there are any tests recommended, they will be scheduled at that time.

New Sleep Clinic Patients:
If you are scheduled at the Sleep Clinic, you will receive a letter and questionnaire prior to your appointment. You will also receive a confirmation call one business day prior to your visit confirming your appointment.
Please arrive at your appointment 15 minutes earlier than your appointment so that we may obtain some preliminary data. It is important that you complete the questionnaire prior to your appointment and bring it with you. Do not mail it to the office. Your appointment will last approximately 1 to 1 ½ hours.
In order to keep waiting time to a minimum for other patients, we may need to reschedule your appointment if you are late or have not completed the questionnaire.
If you cannot make your scheduled appointment, please call our office as soon as possible. In most cases we can offer your appointment to another patient.
